One time it even appeared in the middle of a fight and separated me from the enemies, so it appears they can't get past them either. Turn off darkest settings and save. This only happens at dusk. I had it happen to me in the Cathedral of the Deep, on the walkway in front of the first giant. at 60fps some of the frame dependent animations get bugged.
Lighting the bonfire is fine, you just can't enter the dungeon.

EDIT2: IF YOU GET STUCK IN A WALL OR ANY OTHER WEIRD GETTING-STUCK GLITCH (that isn't fixed by switching glitch types), you can press DELETE to respawn to the nearest checkpoint you activated!
